Provided by Precision Mito, operator of Mito Map
1. Purpose of the pilot
The Mito Map + Flic Early-Access Pilot evaluates whether a compatible physical Flic button can make it easier to record the time when a symptom or other health-related event occurs and add details later in Mito Map. This is an early-access product-usability pilot, not a clinical trial or medical study unless you are separately informed and separately consent to an approved research protocol.
2. Voluntary participation and eligibility
Participation is voluntary. Applying does not guarantee acceptance. You must be at least 18 years old or participate as the parent or legal guardian of the person whose information is recorded. You may decline to answer optional questions or stop participating at any time without losing access to generally available Mito Map features.
3. Pilot period and participant responsibilities
The planned pilot period is approximately eight weeks. If accepted, you agree to make a reasonable effort to:
- follow compatibility, setup, safety, and troubleshooting instructions;
- use the pilot feature only when appropriate and lawful;
- keep your Mito Map and Flic accounts and devices secure;
- verify important records instead of assuming every button press was captured correctly;
- complete brief onboarding, midpoint, and final feedback; and
- report significant technical problems or safety concerns.
4. Hardware purchase and third-party services
You must own or obtain a compatible Flic button and any desired accessories. Precision Mito does not manufacture, sell, warrant, or provide hardware support for Flic products and receives no payment, commission, or referral fee from your purchase. Flic, mobile operating systems, Bluetooth services, and other third-party products are governed by their own terms and privacy practices. Wait for acceptance and compatibility instructions before purchasing hardware.
5. Early-access functionality and technical limitations
The pilot feature may change, be interrupted, or stop working. A press may fail, be delayed, appear more than once, or be associated with the wrong time because of Bluetooth range, permissions, connectivity, battery status, phone settings, operating-system behavior, third-party services, or software defects. Precision Mito does not promise uninterrupted availability, compatibility, accuracy, or a particular outcome. Back up or independently verify information that is important to you.
6. Information collected for the pilot
Depending on how you use the pilot, Precision Mito may collect and process:
- application and eligibility information, including contact details, phone platform, hardware access, availability, and optional accessibility needs;
- pilot event information, including button-press timestamps, event labels, symptoms, severity, notes, triggers, context, edits, and deletion status;
- technical and operational information, including device and browser information, connection state, permissions, delivery status, error logs, diagnostic records, and feature interactions;
- support communications and onboarding, midpoint, and final feedback; and
- agreement version, acceptance date and time, and related consent records.
Do not enter Social Security numbers, payment-card numbers, insurance identifiers, or unrelated medical records into pilot notes or feedback.
7. How pilot information may be used
You authorize Precision Mito and its authorized personnel, contractors, and service providers to access and use pilot information as reasonably needed to operate the feature, deliver and reconcile events, provide support, troubleshoot failures, protect security, evaluate usability and reliability, improve Mito Map, and administer the pilot.
You also authorize Precision Mito to create and use de-identified or aggregated information from pilot data and feedback for product improvement, analysis, validation, internal reporting, presentations, publications, partnership discussions, and commercial development. Precision Mito will not intentionally identify you in a public presentation or publication without separate permission.
8. Optional wearable or health-data connections
This agreement does not authorize a new connection to Apple Health, Health Connect, a wearable platform, an electronic health record, or another health-data source. Any optional connection that imports additional health or wearable data requires a separate, specific permission before activation.
9. Storage, security, access, and service providers
Precision Mito will use reasonable administrative, technical, and organizational safeguards appropriate to the pilot. No storage or transmission method is completely secure. Information may be handled by authorized service providers that support hosting, communications, analytics, security, and technical operations, subject to applicable agreements and law.
10. Retention, export, correction, and deletion
Pilot information may be retained while your account or the pilot is active and afterward as reasonably needed to operate and improve the service, maintain consent and security records, resolve disputes, meet legal obligations, and enforce agreements. You may request a copy of reasonably exportable pilot event data, ask to correct inaccurate information, or request deletion by emailing support@precisionmito.com.
A deletion request may not remove information that must be retained for legal, fraud-prevention, security, backup, or agreement-record purposes. Information already transformed into data that Precision Mito reasonably cannot link back to you, or already included in completed aggregate analyses, may be retained and used.
11. Withdrawal
You may withdraw from the pilot at any time by emailing support@precisionmito.com. Withdrawal ends future pilot participation after the request is processed but does not undo processing that occurred while your consent was active. Unless you also request deletion, information remains subject to the retention terms above. Withdrawing from the pilot does not automatically delete your general Mito Map account.
12. Privacy Policy and Terms of Use
The current Privacy Policy and Terms of Use also apply to your Mito Map account and use of the service. This agreement adds pilot-specific terms. If this agreement expressly gives pilot data a more specific protection than a general policy, the more specific pilot term controls for that pilot data to the extent of the conflict.
13. Not medical care or an emergency service
14. Risks and possible benefits
Risks include missed, delayed, duplicated, or inaccurate records; loss of privacy or confidentiality; frustration; device or battery problems; and decisions made from incomplete information. You may or may not find the workflow helpful. There is no guaranteed personal, medical, or financial benefit.
15. Changes to or end of the pilot
Precision Mito may change, pause, or end the pilot; remove a participant for safety, misuse, incompatibility, or operational reasons; or require acceptance of an updated agreement before continued participation. Material changes will be identified by a new version and effective date. Ending the pilot does not change provisions that by their nature continue, including data retention, de-identified use, disclaimers, and prior authorizations.
16. Questions and contact
For questions, accessibility support, withdrawal, export, correction, or deletion requests, contact support@precisionmito.com.
By checking the agreement box and submitting the pilot application, you confirm that you have read and understand this version of the agreement, have had the opportunity to ask questions, are authorized to agree for yourself or as a parent or legal guardian, and agree to all of its terms. If accepted, your participation is conditioned on this agreement remaining in effect.
